Your therapist keeps written records of your work together. Client records are stored on our Electronic Health Records (EHR) system, Carepaths, which is accessible with your client login information. You have the right to a copy of your file at any time through our EHR and have the right to request through written permission that a copy of your file be made available to anyone including other healthcare providers, attorneys, academic institutions, etc. Any paper records will be maintained in a secure, locked location.
In general, other people cannot access your records without your written consent. It is important to know that in a few instances that protection of your privacy is more limited. For instance, if you are utilizing insurance to pay for services, your insurance company may access some information for determining things like medical necessity of services. Additionally, if you are involved in a legal proceeding and we receive a court order for your records, we are bound by law to release records as outlined in the order. In cases where we are compelled to release any of your treatment information, we will discuss this with you.
